O seu valor vai al\u00e9m da dete\u00e7\u00e3o de uma altera\u00e7\u00e3o molecular: permite tamb\u00e9m quantific\u00e1-la e acompanhar a sua evolu\u00e7\u00e3o. <strong>Desta forma<\/strong>, gera informa\u00e7\u00e3o \u00fatil para o diagn\u00f3stico, a monitoriza\u00e7\u00e3o e a investiga\u00e7\u00e3o.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Esta tecnologia combina a amplifica\u00e7\u00e3o de um alvo espec\u00edfico com a monitoriza\u00e7\u00e3o em tempo real. <strong>Al\u00e9m disso<\/strong>, a sua especificidade, o amplo intervalo din\u00e2mico e a rapidez explicam porque continua relevante ao lado da <strong>PCR digital (dPCR)<\/strong> e da <strong>sequencia\u00e7\u00e3o de nova gera\u00e7\u00e3o (NGS)<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A escolha deve partir da pergunta que se pretende responder. O mais importante \u00e9 a tend\u00eancia observada em amostras consecutivas, analisadas em condi\u00e7\u00f5es compar\u00e1veis e com resultados normalizados. Na leucemia promieloc\u00edtica aguda, por exemplo, a monitoriza\u00e7\u00e3o de <strong>PML::RARA<\/strong> pode apoiar a avalia\u00e7\u00e3o da doen\u00e7a residual mensur\u00e1vel e ajudar a identificar uma recorr\u00eancia molecular. <strong>Da mesma forma<\/strong>, outras fus\u00f5es, muta\u00e7\u00f5es ou rearranjos podem ser estudados se forem informativos para o doente e se o ensaio estiver validado.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Para garantir resultados clinicamente fi\u00e1veis, s\u00e3o essenciais uma fase pr\u00e9-anal\u00edtica controlada, controlos adequados e um gene de refer\u00eancia validado. <strong>Al\u00e9m disso<\/strong>, \u00e9 necess\u00e1rio conhecer a sensibilidade do ensaio e recorrer \u00e0 avalia\u00e7\u00e3o externa da qualidade.<\/p>\n\n\n\n<h5 class=\"c-title-5 wp-block-heading\">qPCR em anatomia patol\u00f3gica: informa\u00e7\u00e3o molecular a partir do tecido<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Na anatomia patol\u00f3gica, a qPCR complementa a avalia\u00e7\u00e3o morfol\u00f3gica e a imunohistoqu\u00edmica. <strong>Assim<\/strong>, acrescenta <strong>informa\u00e7\u00e3o molecular dirigida<\/strong> \u00e0 caracteriza\u00e7\u00e3o da amostra.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A t\u00e9cnica pode ser aplicada a bi\u00f3psias, pe\u00e7as cir\u00fargicas, citologias e material fixado em formalina e inclu\u00eddo em parafina (<strong>FFPE<\/strong>). Atrav\u00e9s de ensaios validados, \u00e9 poss\u00edvel estudar muta\u00e7\u00f5es, transcritos de fus\u00e3o, amplifica\u00e7\u00f5es e altera\u00e7\u00f5es de metila\u00e7\u00e3o. <strong>Al\u00e9m disso<\/strong>, pode ser utilizado ADN de agentes infeciosos associado a determinadas les\u00f5es.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Nos tumores s\u00f3lidos, podem ser avaliadas altera\u00e7\u00f5es em <strong>EGFR, KRAS ou BRAF<\/strong>. No entanto, a escolha do m\u00e9todo deve considerar o tipo de tumor, as recomenda\u00e7\u00f5es cl\u00ednicas em vigor e o n\u00famero de biomarcadores a analisar. <strong>Quando \u00e9 necess\u00e1rio um painel mais amplo<\/strong>, a <strong>NGS pode ser mais eficiente e permitir um melhor aproveitamento do tecido dispon\u00edvel<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As amostras FFPE apresentam desafios espec\u00edficos. A fixa\u00e7\u00e3o pode fragmentar e modificar os \u00e1cidos nucleicos, enquanto a propor\u00e7\u00e3o de c\u00e9lulas tumorais influencia diretamente a sensibilidade do ensaio. <strong>Por esse motivo<\/strong>, podem ser determinantes a revis\u00e3o anatomopatol\u00f3gica, a macro ou microdisse\u00e7\u00e3o quando indicada e a utiliza\u00e7\u00e3o de amplic\u00f5es curtos.
